Senior Case Manager - intake and assessment (remote)

Highbridge Cedar Health Group is looking for a Senior Case Manager. The job centres on intake and assessment for patient scheduling, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with an occasional day on site in Austin. • We reply to every applicant.
