Senior Case Manager - intake and assessment (remote)

Bluepeak Granite Community Partners Group is a Philadelphia charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on intake and assessment. You would join a large organisation of about 292 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with quarterly visits in Philadelphia. • Applications are reviewed on a rolling basis.
