Senior Case Manager - intake and assessment (remote)

Summit Ivory Health Partners is looking for a Senior Case Manager. The job centres on intake and assessment for referrals, and it would suit someone who likes steady, practical work. You would join a large organisation of about 224 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 16 hours a week, mornings. • Remote-friendly, with a monthly team day in Boise. • Applications are reviewed on a rolling basis.
