Senior Case Manager - intake and assessment (remote)

Amberfield Clear Health is looking for a Senior Case Manager. The job centres on intake and assessment for patient scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Kansas City. • Applications are reviewed on a rolling basis.
