Senior Case Manager - intake and assessment (remote)

Bright Basin Health is a healthcare provider in Cleveland with clinics and support teams. We are hiring a Senior Case Manager to take on intake and assessment. We are busy and kind, and we look after each other. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Cleveland. • We will tell you what to expect at each step of the hiring process.
