Senior Case Manager - intake and assessment (remote)

Ironwood Community Partners Group is a nonprofit in Cleveland supporting families with food, housing and job help, and we need a Senior Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Cleveland. • Applications are reviewed on a rolling basis.
