Senior Case Manager - intake and assessment (remote)

Ironwood Linden Care Services supports families and individuals in Memphis with personal care and coordination, and we need a Senior Case Manager to lead our intake and assessment. You would join a large organisation of about 836 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with a monthly team day in Memphis. • We will tell you what to expect at each step of the hiring process.
