Senior Case Manager - intake and assessment (remote)

Amberfield Pike Community Partners is a Chicago charity working with residents on basic needs and opportunity, and we need a Senior Case Manager to lead our intake and assessment. Mission matters here, and so does looking after staff. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for food distribution. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Chicago. • Applications are reviewed on a rolling basis.
