Senior Case Manager - intake and assessment (remote)

Summit Stone Care Services supports families and individuals in Boise with personal care and coordination. We are hiring a Senior Case Manager to take on intake and assessment. You would join a small organisation of about 74 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Boise. • We will tell you what to expect at each step of the hiring process.
