Senior Case Manager - intake and assessment (remote)

We are growing our team at Rivermark Loom Learning Group Partners and adding a Senior Case Manager whose main responsibility will be intake and assessment. We follow the school calendar and we like people who like learning. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for attendance records. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Phoenix. • Applications are reviewed on a rolling basis.
