Senior Case Manager - intake and assessment (remote)

Highbridge Linden Care Services is looking for a Senior Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. Teams support each other, especially on hard days. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Fully remote, with quarterly visits in Chicago. • We reply to every applicant.
