Senior Case Manager - intake and assessment (Providence, RI)

Foxglove Sparrow Care Services is looking for a Senior Case Manager. The job centres on intake and assessment for care plans, and it would suit someone who likes steady, practical work. You would join a small organisation of about 41 people based in Providence. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Providence. • Applications are reviewed on a rolling basis.
