Senior Case Manager - intake and assessment (Providence, RI)

Greenridge Oak Health provides primary and specialty care in Providence, and we need a Senior Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Providence. • We reply to every applicant.
