Senior Case Manager - intake and assessment (Memphis, TN)

Ironwood Forge Care Services Group supports families and individuals in Memphis with personal care and coordination, and we need a Senior Case Manager to lead our intake and assessment. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Memphis. • Applications are reviewed on a rolling basis.
