Senior Case Manager - intake and assessment (Memphis, TN)

Stonegate Quarry Health is looking for a Senior Case Manager. The job centres on intake and assessment for patient education, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Memphis. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Memphis. • Applications are reviewed on a rolling basis.
