Senior Case Manager - intake and assessment (Memphis, TN)

Clearpath Summit Property Management Group looks after rental homes across the Memphis area, and we need a Senior Case Manager to lead our intake and assessment. You would join a small organisation of about 39 people based in Memphis. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for unit turnovers. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Memphis. • We reply to every applicant.
