Senior Case Manager - intake and assessment (Memphis, TN)

Greenridge Umber Health Group provides primary and specialty care in Memphis. We are hiring a Senior Case Manager to take on intake and assessment. We are busy and kind, and we look after each other. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Memphis. • Applications are reviewed on a rolling basis.
