Senior Case Manager - intake and assessment (Madison, WI)

Westbrook Glen Health Partners is looking for a Senior Case Manager. The job centres on intake and assessment for electronic records, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Madison. What you will do • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, Monday to Friday. • On site in Madison, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
