Senior Case Manager - intake and assessment (Louisville, KY)

We are growing our team at Foxglove Juniper Health and adding a Senior Case Manager whose main responsibility will be intake and assessment. You would join a large organisation of more than 1,000 people based in Louisville. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Louisville, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
