Senior Case Manager - intake and assessment (Louisville, KY)

We are growing our team at Rivermark Vista Housing and adding a Senior Case Manager whose main responsibility will be intake and assessment. Collaboration with local agencies is constant. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for lease compliance. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Louisville. • Applications are reviewed on a rolling basis.
