Senior Case Manager - intake and assessment (Knoxville, TN)

Nova Harbor Health provides primary and specialty care in Knoxville. We are hiring a Senior Case Manager to take on intake and assessment. You would join a small organisation of about 26 people based in Knoxville. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Knoxville. • Applications are reviewed on a rolling basis.
