Senior Case Manager - intake and assessment (Kansas City, MO)

Stonegate Harbor Health Group is a healthcare provider in Kansas City with clinics and support teams. We are hiring a Senior Case Manager to take on intake and assessment. You would join a small organisation of about 54 people based in Kansas City. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Kansas City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
