Senior Case Manager - intake and assessment (Jackson, MS)

Fernwood Maple Community Partners Group is a nonprofit in Jackson supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on intake and assessment. We are small enough to know everyone's name. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Jackson. • We will tell you what to expect at each step of the hiring process.
