Senior Case Manager - intake and assessment (Hartford, CT)

Fernwood Harbor Care Services is a social-care provider serving older adults and people with disabilities around Hartford, and we need a Senior Case Manager to lead our intake and assessment. You would join a small organisation of about 167 people based in Hartford. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Hartford. • We will tell you what to expect at each step of the hiring process.
