Senior Case Manager - intake and assessment (Hartford, CT)

Nova Granite Housing is a nonprofit housing provider in Hartford, and we need a Senior Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident services. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident services.
