Senior Case Manager - intake and assessment (Grand Rapids, MI)

Foxglove Summit Community Partners is a nonprofit in Grand Rapids supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on intake and assessment. We are small enough to know everyone's name. What you will do • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Grand Rapids. • We will tell you what to expect at each step of the hiring process.
