Senior Case Manager - intake and assessment (Grand Rapids, MI)

Ironwood Birch Care Services Group is a social-care provider serving older adults and people with disabilities around Grand Rapids, and we need a Senior Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Grand Rapids. • We reply to every applicant.
