Senior Case Manager - intake and assessment (Fort Worth, TX)

Clearpath Ashton Civic Services supports public agencies in Fort Worth with budgeting, records and programmes. We are hiring a Senior Case Manager to take on intake and assessment. What you will do • Help with applications and appointments for constituent requests. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Fort Worth, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
