Senior Case Manager - intake and assessment (Fort Worth, TX)

Northwind Sparrow Care Services Group is a social-care provider serving older adults and people with disabilities around Fort Worth. We are hiring a Senior Case Manager to take on intake and assessment. The work is personal and so is the culture. What you will do • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Fort Worth. • Applications are reviewed on a rolling basis.
