Senior Case Manager - intake and assessment (Fort Worth, TX)

Westbrook Oak Health Group provides primary and specialty care in Fort Worth, and we need a Senior Case Manager to lead our intake and assessment. You would join a small organisation of about 101 people based in Fort Worth. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Fort Worth. • Applications are reviewed on a rolling basis.
