Senior Case Manager - intake and assessment (Fort Worth, TX)

Highbridge Wren Care Services supports families and individuals in Fort Worth with personal care and coordination. We are hiring a Senior Case Manager to take on intake and assessment. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Fort Worth. • Applications are reviewed on a rolling basis.
