Senior Case Manager - intake and assessment (Fort Worth, TX)

Fernwood Lantern Community Partners is a Fort Worth charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on intake and assessment. We are small enough to know everyone's name. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for donor records. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Fort Worth. • We reply to every applicant.
