Senior Case Manager - intake and assessment (El Paso, TX)

Stonegate Mill Care Services Group supports families and individuals in El Paso with personal care and coordination, and we need a Senior Case Manager to lead our intake and assessment. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in El Paso. • We reply to every applicant.
