Senior Case Manager - intake and assessment (Dallas, TX)

Foxglove Meridian Health is looking for a Senior Case Manager. The job centres on intake and assessment for referrals, and it would suit someone who likes steady, practical work. You would join a small organisation of about 100 people based in Dallas. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Dallas, with some flexibility after the first few months. • We reply to every applicant.
