Senior Case Manager - intake and assessment (Columbus, OH)

Ironwood West Care Services Group provides in-home and community support in Columbus. We are hiring a Senior Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Columbus, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
