Senior Case Manager - intake and assessment (Colorado Springs, CO)

Bluepeak Stone Care Services Partners is a social-care provider serving older adults and people with disabilities around Colorado Springs, and we need a Senior Case Manager to lead our intake and assessment. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
