Senior Case Manager - intake and assessment (Cleveland, OH)

Lumen Vista Housing is looking for a Senior Case Manager. The job centres on intake and assessment for waiting lists, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Cleveland. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for waiting lists. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with waiting lists. • Bilingual English and Spanish.
