Senior Case Manager - intake and assessment (Chicago, IL)

Lumen Lantern Property Management Partners is looking for a Senior Case Manager. The job centres on intake and assessment for lease renewals, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for lease renewals. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease renewals. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Chicago. • Applications are reviewed on a rolling basis.
