Senior Case Manager - intake and assessment (Charleston, SC)

We are growing our team at Bluepeak Sable Community Partners and adding a Senior Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Full-time, on a regular day shift. • On site in Charleston. • We reply to every applicant.
