Senior Case Manager - intake and assessment (Buffalo, NY)

Bright Quarry Care Services Group provides in-home and community support in Buffalo, and we need a Senior Case Manager to lead our intake and assessment. You would join a large organisation of more than 1,000 people based in Buffalo. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Buffalo. • We reply to every applicant.
