Senior Case Manager - intake and assessment (Boise, ID)

Harborlight Yarrow Health is a healthcare provider in Boise with clinics and support teams, and we need a Senior Case Manager to lead our intake and assessment. You would join a small organisation of about 13 people based in Boise. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling.
