Senior Case Manager - intake and assessment (Boise, ID)

We are growing our team at Foxglove Orchard Care Services and adding a Senior Case Manager whose main responsibility will be intake and assessment. We train and promote from within. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, Monday to Friday. • On site in Boise, with some flexibility after the first few months. • We reply to every applicant.
