Senior Case Manager - intake and assessment (Boise, ID)

Greenridge Harbor Care Services provides in-home and community support in Boise, and we need a Senior Case Manager to lead our intake and assessment. The work is personal and so is the culture. What you will do • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Boise. • Applications are reviewed on a rolling basis.
