Senior Case Manager - intake and assessment (Boise, ID)

Lumen Cross Health Group runs clinics and patient services around Boise. We are hiring a Senior Case Manager to take on intake and assessment. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Boise. • We reply to every applicant.
