Senior Case Manager - follow-up and outcomes (Tucson, AZ)

Foxglove Ivory Health Group provides primary and specialty care in Tucson. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Tucson. • We reply to every applicant.
