Senior Case Manager - follow-up and outcomes (Tucson, AZ)

We are growing our team at Summit Ridge Learning Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Families are partners. What you will do • Help with applications and appointments for learning materials. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Tucson. • We reply to every applicant.
