Senior Case Manager - follow-up and outcomes (Tucson, AZ)

Summit Linden Health Group is looking for a Senior Case Manager. The job centres on follow-up and outcomes for patient scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Tucson, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
