Senior Case Manager - follow-up and outcomes (Tucson, AZ)

Redstone Forge Care Services Partners provides in-home and community support in Tucson, and we need a Senior Case Manager to lead our follow-up and outcomes. We train and promote from within. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Tucson. • We will tell you what to expect at each step of the hiring process.
