Senior Case Manager - follow-up and outcomes (Tucson, AZ)

Foxglove Willow Community Partners Group is a Tucson charity working with residents on basic needs and opportunity, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Tucson. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Tucson, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
