Senior Case Manager - follow-up and outcomes (Toledo, OH)

We are growing our team at Northwind Cedar Care Services and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client records. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Toledo. • We reply to every applicant.
